As of March 1, 2002, the Roche Blood Glucose Monitoring Systems and Test Strips will be our preferred products in this category.
For new blood glucose testers the easiest and fastest way to obtain a Roche blood glucose monitor, is to write a prescription for the patient or phone one in to a participating pharmacy. The toll free telephone number previously supplied to physician offices for new testers will no longer be available with the introduction of this more convenient process. Your patient will be able to obtain one of the following, at a participating pharmacy, with no copay:
The companion test strips for these monitors are available at participating pharmacies and are covered by the Plan at the preferred-level Rx copayment. These are:
All brands of blood glucose test strips other than Accu-Chek will be considered non-preferred with a non-preferred co-pay. For those with a Major Medical or single tier prescription drug plans, any blood glucose test strips other than Accu-Chek will not be covered.
In the instances where more than one member of a family at the same address requires a blood glucose monitor, only one monitor per address can be obtained via this toll-free telephone number. Other family members may obtain their needed monitors with a physicians prescription at a participating pharmacy. At this time, the Accu-Chek Compact is available only through participating pharmacies.
Members who need an Accu-Chek monitor replaced due to damage should call the toll-free telephone number listed on the back of their monitor.
