Important News About Your BlueCare Coverage
Your BlueCare® coverage offers the benefits and services you need to
get and stay healthy. Occasionally, we make improvements to be sure you are
getting the most comprehensive and the most cost-effective health care coverage
available. From time to time, we also make operational changes to how you can
access care or to our policies. The following is the most recent information
you need to be sure you have access to all your covered benefits and services.
New Group Enrollment Application/Change Form - January 2008
From time to time BlueCross of Northeastern Pennsylvania updates the Enrollment
Application/Change Form for Group Coverage. We’ve listened to our groups, and a
new form was designed to make it as easier for both the group administrator and
the enrollee to complete.
The top section is to be completed by the Group Administrator. It lists all
group information needed for enrollment.
Sections 1 and 2 are applicant and dependent information which need to be
completed in full. If there is any information not completed, the application
will be returned unprocessed. The purpose is to be sure that all accurate and
complete information is entered into our system to allow for processing of
enrollment and eventual claim payment. Section 2 is continued on the reverse
side of the application and must also be completed in full by the applicant.
Section 3 is general applicant information which needs to be completed. Section
4 is applicable for enrollment in BlueCare HMO Plus only and Section 5 needs to
be read and signed by both the applicant and the Group Administrator.
Enrollment and Billing will continue to accept the old application forms for run
out period which will not extend past June 1, 2008. The most recent
application/change form is dated January, 2008 and that date is located in the
bottom left-hand corner of the form. If you need to print the form, please
visit the Group Administrator page of our website or call your Account Support
Representative for a supply.
BlueCare POS is now BlueCare HMO Plus
A New Name, With All the Same Benefits and Services You Deserve.
Effective January 1, 2008, Blue Cross of Northeastern Pennsylvania will change
the name of the BlueCare POS health care plan to BlueCare HMO Plus.
The name will change to more accurately reflect the benefits and services
offered with this open-access HMO health care coverage plan.
Rest assured that only the name will change. There is no change in benefits or
services you will receive. You will receive a new ID card within the next few
weeks. Please destroy your old card and begin using the new ID card
immediately. And, contracts will be updated with the new name and you will
receive an updated contract upon your group’s renewal for 2008.
With BlueCare HMO Plus, you will continue to receive the same quality,
comprehensive health care coverage from the most trusted name in health care
for nearly 70 years.
