Group Administrator Forms
From time to time, you or your employees will need to complete forms, applications and disclosures in order to use your health care coverage plan.  The following is a list of the most common forms. Your group administrator manual and BlueCare handbook will explain when you will need to use these.

Simply click on whichever form you need, print and complete it and return it to us.  And, when applicable, advise your employees to do the same. Don’t forget to always keep a copy for your record.
