BlueCare Member Forms
From time to time, you will need to complete forms, applications and disclosures in order to use your health care coverage plan.  The following is a list of the most common forms. Your handbook will explain when you will need to use these.

Simply click on whichever form you need, print and complete it. Then, depending on the instructions, either give the form to your group administrator or mail it back to us. Don’t forget to always keep a copy for your record.
Group Health Plan Forms

Use the following forms if you are enrolled in the group health care coverage plans:  BlueCare Traditional, BlueCare Comprehensive, BlueCare QHDPPO, BlueCare PPO, BlueCare POS or BlueCare HMO.
