Boys and Girls Clubs of Northeastern Pennsylvania
$10,000.00 - “Get Real About Violence” Life Skills Training
The Boys and Girls Clubs of Northeastern Pennsylvania will partner with substance abuse prevention agencies in Lackawanna and Luzerne Counties to offer violence prevention and life skills training to 130 youngsters, ages 6 through 13. After-school workshops led by certified education specialists will teach participants age-appropriate strategies for eliminating high-risk behaviors.
Carbon County Partners for Progress
$10,000.00 - Healthy Smiles, Happy Kids Mobile Dental Van
The Healthy Smiles, Happy Kids Mobile Dental Van was launched to address the lack of access to dental services for un/underinsured youngsters in Carbon County. The van travels to elementary schools throughout the county and provides dental exams, cleaning and sealant services, and restorations to qualified kids. The Foundation’s grant will enable 200 new un/underinsured children to be visited twice during the coming year.
Carbon Monroe Pike Drug and Alcohol Commission
$10,080.00 - Substance Abuse Case Management Program
The Carbon Monroe Pike Drug and Alcohol Commission has developed a comprehensive, office-based detoxification and maintenance program for clients needing services for opiate addiction. Foundation funding will help provide behavioral case management services for 125 low-income clients during the first three months of the year-long program, increasing the likelihood of success and the achievement of long-term health improvements.
Clinton County Healthy Communities
$16,000.00 - Clinton County Community Dental Clinic
The Clinton County Community Dental Clinic was launched in response to an increased demand for dental services for un/underinsured youngsters in Clinton County. With this grant, the Clinic will be able to provide free primary dental services and prevention education to 250 un/underinsured pediatric patients and their families.
Commission on Economic Opportunity
$73,600.00 - Diabetes Screenings and Education for the Uninsured
The Diabetes Screenings and Education for the Uninsured Program is a comprehensive one-year initiative designed to address diabetes and prediabetes recognition, education, and self-management among un/underinsured Luzerne County residents. With Foundation funding, the program will reach 5,000 individuals served by the six free health clinics in Luzerne County in the coming year.
Community Medical Center Healthcare System
$61,500.00 - Circle of Care – Prenatal Case Management Program
The Circle of Care located at CMC is a collaboration between Maternal and Family Health Services (MFHS) and numerous community health partners to promote healthy birth outcomes by providing comprehensive health care to un/underinsured pregnant and postpartum women. With the number of high-risk pregnancies among this population rising dramatically in Lackawanna County, Foundation funding will be used to provide intensive prenatal case management services to an estimated 300 high-risk patients in the coming year.
Diakon Lutheran Social Ministries
$4,725.00 - Girls on the Run – Life Skills Training Program
Diakon will partner with elementary and middle schools in Lycoming County and targeted community partners to offer Girls on the Run (GOTR) Life Skills Training to 224 young females in the 3rd through 8th grades. GOTR is an international prevention curriculum that encourages pre-teen girls to develop self-respect and healthy lifestyles by eliminating high-risk behaviors.
Family Service Association of Wyoming Valley
$8,100.00 - Parent-Child Home Program
Since 2002, Family Service Association has been delivering the Parent-Child Home Program (PCHP), a proven early childhood development program, to Luzerne County families challenged by poverty, limited education and low literacy. The Foundation’s grant will help the agency broaden the program and deliver additional parenting, health, and nutrition education services to 100 high-risk families over the next year.
Greater Scranton YMCA
$20,000.00 - C.A.T.C.H. Kids Club
In an effort to combat childhood obesity, the Greater Scranton YMCA will add the CATCH (Coordinated Approach To Child Health) curriculum to its existing summer program for low-income youth. CATCH will ensure that at least 120 minutes of daily physical activity and weekly nutrition education sessions are provided to 100 low-income or physically at-risk children for 12 weeks this summer.
Infant Development Program
$10,000.00 - P.A.C.T. Parenting Program
Infant Development Program will partner with Clinton County Children and Youth and local school districts to provide the P.A.C.T. (Parenting at a Challenging Time) Program to 60 at-risk families in Clinton County. The parenting education programs are designed to improve the physical health and well-being of, and prevent behavior problems in, children below the age of 10 from high-risk environments.
Marywood University
$9,800.00 - Speech-Language-Hearing Screenings for Children
The Marywood University Speech-Language-Hearing Clinic will provide free speech, language and audiology screenings for pre-school children at 18 Head Start Centers across Lackawanna County. An estimated 825 low-income youngsters will be screened in the coming year, and participants identified with early hearing loss and/or speech-language delays will be referred for follow-up services.
MetroAction, Inc.
$10,000.00 - Kindergarten Readiness Nutrition Education Program
MetroAction, Inc. is partnering with the United Way of Lackawanna County’s Success by Six Program and the Valley View, Riverside, and Forest City School Districts to provide the Kindergarten Readiness Nutrition Education Program. The program will educate 20 childcare providers and 200 low-to-moderate income families on ways to foster healthy eating as children transition from daycare/home care to kindergarten.
North Penn Comprehensive Health
$25,100.00 - Project Reach
North Penn Comprehensive Health Service will use Foundation funding to provide free diagnostic testing to at least 550 un/underinsured patients at six community-based clinics throughout Tioga County.
North Penn Comprehensive Health Services
$15,500.00 - I Am Moving, I Am Learning
North Penn Comprehensive Health Services will partner with all 22 Head Start centers in Bradford and Tioga Counties to offer “I am Moving, I am Learning,” a nationally known program designed to address childhood obesity in Pre-K children. The program will engage 374 low-income youngsters in daily physical activities and nutrition lessons during the 2009/10 school year.
North Penn Legal Services
$6,525.00 - Living Will Outreach Project
North Penn Legal Services designed its “Living Will Outreach Program” to provide no-cost education, legal advice and document preparation services on living wills to low-income, elderly individuals in five rural counties. More than 185 seniors, at or below 125% of the federal poverty guidelines, will benefit from the program.
Northeast Regional Cancer Institute
$50,000.00 - Increasing Breast Cancer Screening Compliance
To encourage women over 60 to follow recommended breast cancer screening standards, the Northeast Regional Cancer Institute will partner with area providers and social service agencies to offer free educational programs to 2,000 age-targeted women at community sites across six counties. Topics will include the risk factors of breast cancer, the importance of routine screening/early detection in successful treatment, and individualized information on screening sites, payment assistance, and access to transportation.
Partners in Family and Community Development
$7,000.00 - Bradford County Nurse-Family Partnership Expansion
The Nurse-Family Partnership (NFP) Program provides health education, life skills training, developmental screenings and other services to low-income expectant mothers and new moms and their babies. The Bradford County NFP is at capacity and has a waiting list for services. Foundation funding will help offset the mandatory training costs for new personnel and allow 20 new families to be served in the coming year.
Penn State Cooperative Extension in Pike County
$7,300.00 - Up for the Challenge: Lifetime Fitness, Healthy Decisions
Penn State Cooperative Extension in Pike County will partner with six existing youth summer programs throughout the county to offer nutrition and fitness education to 75 children, ages 8 to 12. The program will teach children how to identify healthy foods, prepare healthy snacks, and incorporate exercise into their daily routines.
Pennsylvania Elks Major Projects, Inc.
$10,000.00 - Home Service & Medical Care Coordination
Pennsylvania Elks Major Projects, Inc. provides special needs adults with health and support services under the guidance of a registered nurse. Foundation funding will support home visitations and medical care coordination for 100 special needs individuals across 13 counties in the coming year.
Pennsylvania Environmental Council, Inc.
$7,680.00 - Community Garden Nutrition Workshops
The Pennsylvania Environmental Council and the City of Wilkes-Barre Health Department will use Foundation funding to begin Nutrition Workshops this summer for participants at two community garden sites. More than 250 low-income youth and adults will learn about the health benefits of a well-rounded diet, the importance of increasing their use of fresh fruits and vegetables, and the importance of physical activity.
Pocono Family YMCA
$16,500.00 - CATCH After School Program
The Pocono Family YMCA will use Foundation funds to add the CATCH (Coordinated Approach To Child Health) curriculum to its existing after-school program. CATCH will ensure that at least 180 minutes of weekly physical activity and nutrition education are provided to 120 low-income or physically at-risk children in the coming year.
Pocono Healthy Communities Alliance
$9,300.00 - Healthy Start Screening Program
Pocono Healthy Communities Alliance (PHCA) will coordinate multiple partners to provide the Healthy Start Screening Program to 400 low-income Monroe County children, ages 3 to 5. The program offers free screenings to help detect health, social-emotional and learning disabilities in these children, and provides referrals and case management services if intervention is needed.
Rural Health Corporation of Northeastern Pennsylvania Rural Health Corporation of Northeastern Penn
$20,000.00 - Dental Services for the Uninsured
Rural Health clinics serve approximately 18,000 un/underinsured patients each year, many of whom need dental services. This grant will help the clinics increase their inventory of commonly needed dental supplies and tobacco prevention educational materials to serve an additional 1,000 un/underinsured adults in the coming year.
Safe Haven of Pike County
$8,500.00 - Safe Haven School Program
Safe Haven of Pike County will partner with 11 schools in two school districts to offer the Safe Haven School Program to more than 3,600 youngsters. The program provides age-appropriate personal safety education to K through 12 students, and creates an opportunity for the trained instructors to identify potential abuse among the participants.
Serento Gardens
$7,000.00 - MED-ED Workshops for Seniors
MED-ED Workshops are free weekly programs that are offered to seniors and their caregivers at targeted senior centers and housing complexes in the Greater Hazleton area. Topics include safe and appropriate medication practices, how to recognize and manage common side effects, and how to identify the signs of alcohol abuse, which are often hidden in seniors. At least 150 seniors will participate in the coming year.
TREHAB, Inc.
$16,000.00 - Workshops on the Medical Consequences of Substance Abuse
TREHAB, Inc. will use grant funding for a comprehensive workshop series addressing the health consequences of substance abuse and its connection to conditions such as cardiovascular disease, stroke, cancer, diabetes, and lung disease. The workshops will be offered to 400 at-risk individuals and 250 health care professionals throughout Susquehanna County.
Volunteers in Medicine
$75,000.00 - Diagnostic Services for the Uninsured
The Volunteers in Medicine (VIM) Clinic serves the working uninsured of Luzerne County. VIM will use Foundation funding to provide necessary diagnostic testing and lab services to 1,700 patients in the coming year.
Wayne County YMCA
$7,700.00 - “Fit and Functional” – Adult Wellness Program
The Wayne County YMCA will partner with local community health partners to offer the “Fit and Functional” program to 60 low-income, physically at-risk adults in Wayne County. The program integrates nutrition education and physical activity to teach these individuals how to incorporate healthy behaviors into their daily routines.
Wilkes University
$20,000.00 - Pharmaceutical Supplies for the Uninsured
The Pharmaceutical Access Program is a collaborative effort between Wilkes University’s Nesbitt School of Pharmacy, the Commission on Economic Opportunity, and six free health clinics in Luzerne County. The program provides the clinics with access to a pharmaceutical bank, medical supplies and diagnostic testing supplies, pharmaceutical education and counseling, and case management services. The Foundation’s grant will be used to increase the inventory of medications needed to treat 6,000 un/underinsured individuals in the next year.
Wilkes-Barre Family YMCA
$8,750.00 - CATCH After School Program
The Wilkes-Barre Family YMCA will use Foundation funds to add the CATCH (Coordinated Approach To Child Health) curriculum to its existing after-school program. CATCH will ensure that at least 45 minutes of daily physical activity plus nutrition education are provided to 30 low-income or physically at-risk children in the coming year. In addition, the YMCA will provide bi-monthly healthy lifestyle education sessions for the parents and caregivers of these children.
Women’s Resources of Monroe County, Inc.
$10,000.00 - Un Lugar Seguro (A Safe Place)
Women’s Resources of Monroe County has experienced an increase in the number of non-English speaking victims of domestic and sexual violence. In response, the Un Lugar Seguro (A Safe Place) Initiative aims to increase education and awareness about domestic and sexual violence among the county's Spanish-speaking population in a culturally appropriate context. With the Foundation’s help, Un Lugar Seguro will reach an additional 75 Spanish-speaking clients and their family members in the coming year.
Wyoming Valley Alcohol and Drug Services, Inc.
$49,990.00 - Alcohol, Tobacco, and Other Drugs (ATOD): Prevention Through Education Prevention Through Education
Wyoming Valley Alcohol and Drug Services, Inc. will partner with middle schools in 12 school districts across two counties to provide substance abuse prevention education to 1,800 students and 55 teachers in the 5th through 8th grades. Education sessions facilitated by trained prevention educators will cover topics such as alcohol and tobacco use, abuse, and addiction, culture, media and technology, and over-the-counter prescription drug dangers.
Wyoming Valley Children’s Association
$11,180.00 - Orthopedic Clinic for Children
Through its Orthopedic Clinic, Wyoming Valley Children’s Association (WVCA) provides orthopedic evaluations for young people, birth to 21 years of age, with abnormalities such as postural disorders, bone/joint conditions or abnormalities that are typical of children who have developmental disabilities. Funding will be used to expand clinic coordination services and help the clinic serve 140 un/underinsured children in the coming year.
