Catastrophic Case Management Program

Blue Cross of Northeastern Pennsylvania offers Catastrophic Case Management in which a Care Coordinator can act as your advocate to coordinate the care you need.

An assessment is available for individuals who:

• Require high-tech or extensive home care
• Are terminally ill
• Have sustained traumatic injury
• Require frequent hospital admissions
• Have a long-term illness
