Heart Failure Management Program
Approximately 5 million people in this country have heart failure (HF) and more
than 550,000 are diagnosed with HF for the first time each year. Heart failure
means that your heart is not pumping enough blood and therefore, enough oxygen,
to meet your body’s needs.
Blue Cross of Northeastern Pennsylvania offers its Heart Failure Management
Program to members/insureds to help improve their quality of life. If you have
been diagnosed by your doctor with heart failure or have the following
symptoms, take advantage of the valuable resources that Blue Cross of
Northeastern Pennsylvania offers.

Frequent cough or wheezing

Bloating

Swollen feet, legs and ankles

Sudden weight gain

Nausea or loss of appetite

Fatigue or weakness

Rapid or irregular heartbeat
Our Heart Failure Management Program will help you with symptom management as
well as support you as you take the healthy actions you need to improve the
quality of your life. A program Care Coordinator will offer you the education,
resources and phone support you need to make a difference in your health.
