Lifestyle Management Program Helps You Manage Your Health

The best way to avoid getting sick is to take a proactive role in your health by practicing healthy habits such as:

  • Eating healthy meals
  • Exercising regularly as appropriate for your health*
  • Getting annual checkups with your doctor
  • Receiving preventive screenings as recommended, such as mammograms, an annual flu shot, colorectal cancer screenings, etc.
  • Understanding your family’s health history so you know what your risks might include

To get started, visit MyHealth Solution to take a confidential health assessment which will give you a better picture of your general health and identify any personal risks you may have for future problems. Based on this, you will be able to take advantage of a wide range of personalized wellness and education tools that will make it easier for you to make the right choices and improve your well-being.

Whether you are feeling stress, are carrying some extra weight or need help forming an exercise plan, the Care Coordinators of our Lifestyle Management Program can help you.

  • Call 1-866-262-4764, weekdays, 8 a.m. to 8 p.m. to speak with a health care professional. They can offer you support as well as education and resources to help you with stress, nutrition, fitness, high blood pressure, cholesterol and other issues related to your lifestyle habits.
  • If you know that you have a chronic or more serious health condition, you should seek help from one of our health management or care management programs listed to the left of this page. Click on any program of interest for more information.

*The CDC says adults should engage in moderate level activities for at least 30 minutes on five or more days of the week. Examples of moderate activities include: walking briskly, mowing the lawn, dancing, swimming, or bicycling on level ground. Before starting any new exercise program, you should speak with your doctor to determine what is best for you.
