BlueCare^® HMO is our health maintenance organization plan. The plan is designed to promote and protect your health by emphasizing preventive care through a large network of health care providers. At the same time, the HMO includes several important cost control measures.

BlueCare HMO Plus

First Priority Health

BlueCare HMO Plus incorporates all the advantages of a managed care program while providing a greater element of choice. Higher benefits are paid for care provided in the network or with approval (prior authorization), while you are free to have services from BlueCard^® providers directly at lower levels of reimbursement.

BlueCare EPO

First Priority Life Insurance Company ^®

A preferred provider product with an exclusive network, BlueCare EPO is designed based on where you live and/or work. The preferred provider networks differ depending on the plan design of the product to ensure your coverage includes the most comprehensive provider network to meet your needs. There is no need to choose a Primary Care Physician. Services may be subject to copayments, deductible and/or coinsurance. Coverage from non-preferred providers is only available in an emergency.

BlueCare PPO

First Priority Life Insurance Company

A Preferred Provider Organization, BlueCare PPO features an impressive combination of choice and cost control features. You will enjoy maximum benefit levels by choosing health care providers from our preferred provider network. At the same time, you can choose to receive care from outside the network at lower levels of coverage. Services may be subject to a deductible and coinsurance.

BlueCare QHD PPO

First Priority Life Insurance Company

BlueCare Qualified High Deductible PPO is designed to be offered in conjunction with a health savings account (HSA). This product offers freedom of choice with the ability to receive care from health care providers in our preferred provider network at the highest level of benefits or to receive care outside the network at a reduced level of coverage.

BlueCare Comprehensive

First Priority Life Insurance Company

BlueCare Comprehensive consists of hospitalization, medical/surgical and major medical coverage all wrapped up in one complete benefit package. You will receive benefits at the highest level when contracting health care providers are used. Services are subject to a deductible and coinsurance.

BlueCare Traditional

First Priority Life Insurance Company

Our BlueCare Traditional health care plan offers quality medical coverage including hospitalization, medical/surgical and major medical services. With BlueCare Traditional, you will receive benefits at the highest level when performed by contracting health care providers. Services may be subject to a deductible and coinsurance when non-contracting providers are used.

BlueCare Senior

Blue Cross of Northeastern Pennsylvania
and Highmark Blue Shield

BlueCare Senior has been designed to supplement the federal Medicare program. It combines Blue Cross benefits for covered hospital expenses with Blue Shield benefits for covered doctor services. All actively-employed group members age 65 or older whose employers have less than 20 full and/or part time employees are eligible for BlueCare Senior. You must also be enrolled in Medicare Part A and Part B. If you are receiving Medicare due to a disability, the 20-employee rule changes to 100 employees. You are also eligible for BlueCare Senior if you are retired, joining a retiree group formed by your employer and you are enrolled in Medicare Part A and Part B.
