$6,737.00 - Case Management Services
With this grant, the agency’s case workers will be able to provide 425 hours of specialized health and safety-related evaluation, scheduling, and appointment assistance to 200 women and children experiencing homelessness.
$8,000.00 - Adaptive Exercise Program
A professional fitness instructor will provide an onsite Adaptive Exercise Program to 160 adults with disabilities at four day program locations. Staff members at each location will also be trained to continue the program with their clients after the grant period concludes.
$5,000.00 - Youth Mental Health First Aid
Youth Mental Health First Aid (YMHFA) is an evidence-based program developed by AmeriCorps and designed to train individuals who work with adolescents to better understand and recognize mental health and crisis situations. A certified instructor will provide YMHFA training to 150 community participants over the coming year.
$5,111.00 - Parents as Teachers Program
Certified instructors will visit the homes of 75 at-risk families twice a month during the coming year to provide guided parenting education and health and developmental screenings to all children under the age of five.
$8,836.00 - Arthritis Aquatics Program
The YMCA/YWCA will waive the fee for, and expand, its existing Arthritis Aquatics Program to serve 120 individuals in the coming year, with special outreach to neighboring seniors. A certified instructor will lead the group in specialized exercises designed to increase strength and flexibility and reduce pain.
$7,000.00 - LCCC Dental Clinic for the Uninsured
The Luzerne County Community College Dental Clinic will use this grant to purchase the supplies necessary to provide preventive dental services to 520 uninsured children and adults in the coming year.
$9,975.00 - Digital Mammography Screening Program for the Un/Underinsured
With this grant, Palmerton Hospital will provide free mammogram screenings to 95 un/underinsured women in the coming year. Participants will also be educated about the importance of yearly screenings.
$4,300.00 - Dental Services for Uninsured & Underinsured Children
Pocono Medical Center’s Valerie M. Hodge Memorial Pediatric Dental Center will purchase the supplies needed to provide dental exams, preventive care, and oral hygiene education to 1,600 un/underinsured children.
$8,089.00 - Cardiovascular Screening & Awareness for Older Adults
A registered nurse and trained public health professional will educate 200 older adults about cardiovascular disease risk factors and prevention. All participants will receive blood pressure screenings and anyone determined to be at high-risk will receive additional monitoring and support.
$8,000.00 - Pharmaceutical Support for the Uninsured
The University of Scranton’s Edward R. Leahy Center Clinic for the Uninsured will purchase the pharmaceuticals, medical supplies, and immunizations needed to serve 750 uninsured individuals in the coming year.
$7,056.00 - Ruth’s Place Therapeutic Group Sessions
Ruth’s Place, a program of Volunteers of America of Pennsylvania, will offer therapeutic group sessions three times a week for the next year to help 300 women experiencing homelessness develop healthy living, coping, decision-making, and life skills.
$9,576.00 - Diabetes Self-Management Program
This grant will enable Wayne Memorial Community Health Centers to extend a proven diabetes self-management curriculum to patients at seven community-based clinic sites. A Certified Diabetes Educator will provide self-management instruction and post-program follow-up calls to 210 diabetic patients and support family members.
$8,760.00 - LIVESTRONG at the YMCA
LIVESTRONG at the YMCA is an evidence-based curriculum offered at Y’s across the country. In Wilkes-Barre, specially trained YMCA staff will provide customized physical activity and counseling to 48 adult cancer survivors.
$9,929.00 - Safety Planning Program
With this grant, counselor advocates will be able to provide 600 hours of safety planning assistance to 400 women at risk for domestic violence and/or sexual assault.
