Blue Health Solutions named “Best Valid Disease Management Program” in Pennsylvania
November 17, 2011, Wilkes-Barre, PA — Blue Health Solutions, the health and wellness program available at no additional cost to members and customers of Blue Cross of Northeastern Pennsylvania (BCNEPA), has been recognized by a national certification agency for its program excellence.
The Disease Management Purchasing Consortium (DMPC), the industry’s leading outcomes certification organization,recently recognized Blue Health Solutions as having the “Best Valid Disease Management Program” in Pennsylvania. DMPC evaluates the outcomes of disease and population management programs, and is the largest provider of certifications for valid savings measurement for health plans and employers.
According to the Care Continuum Alliance (CCA), disease management is a system of coordinated health care interventions and communications for patients with conditions in which self-care and management efforts are significant.
“Utilizing tools such ashealth coaches and other online tools provided by Blue Health Solutions, we are able to offer our clients, their employees, and covered family members the support they need to achieve their wellness goals, manage chronic diseases and engage in more healthy behaviors,” said A. Paul Holdren, senior vice president and chief sales officer for BCNEPA.
And through Blue Health Solutions, BCNEPA members that have been diagnosed with a chronic illness have access to experienced health coaches, such as nurses, dietitians and other health professionals. The coach works with the member to develop a personalized plan to help manage his or her condition.
“Our success comes from the individual approach our care coordinators take with each member,” said Stuart Segal, senior vice president, integrated health services. “Creating a condition-specific management plan that reflects the member’s personal health status and goals helps the member achieve better outcomes for managing their condition.”
In reviewing the outcomes of disease management programs in the Commonwealth, DMPC president Al Lewis had high praise for Blue Health Solutions.
“As of this writing, every other valid non-Medicaid program in Pennsylvania is producing less impact than the Blue Health Solutions program.”
Blue Health Solutions achieved its results by bringing focus to health status improvement and impacting the inpatient admission and emergency room visit event rates for diabetes, coronary artery disease, chronic obstructive pulmonary disease (COPD) and congestive heart failure.
“These results show that there is real value in disease management programs for the individual members and for employers,” said Segal. “When chronic conditions are better managed, our members can improve their health status and quality of life. And the data from employees who are engaged in a wellness or disease management program trend toward lower health care costs.”
